Facility Use Form (Legal Paper)

Board Policy requires any organization that is not an official school club to complete a Facility Use Form.  This form must be submitted to Beverly Jackson no less than ten(10) working days prior to the event.

1.  Download Form and complete Information Section

2.  Meet with School Principal to get School Permission, arrange for custodial services, and hire security.  If School Fees are waive (not for profit only) Principal must initial document.

3.  You must include insurance certificate with form, two checks (First check if for School Fees and second check is for JCSB Fees.) and deliver or mailed to Facilities Director, at Jackson County School Board Office, 2903 Jefferson Street, Marianna, FL  32446.

4.  Once insurance is verified or purchased, form will be sent to School with signatures. 

5.  School will notify school that form is complete and fees are paid.
